I am directed to inform you that the Canadian and Imperial Governments are issuing certain Memorials to the next of kin of our state soldiers.

In order that the proper disposition of these Memorials may be ascertained, will you please complete the enclosed Army Form P-64 and return it to this Directorate, at your earliest convenience. Special care should be taken to fill in the full names, ages, address and dates of death of all relatives in the degrees call for therein.
